I reserve 45-60 minutes for each appointment with a client unless otherwise discussed with client. If you are unable to keep a scheduled appointment, please notify me as soon as possible. Late cancellations make it difficult to offer the appointment time to someone else. Therefore, missed appointments and cancellations made less than one full business day (48 hours) in advance of the scheduled appointment will result in you being charged in full for your reserved appointment time. Payment is expected at the time of service unless other arrangements have been agreed upon. The cost of therapy services is your responsibility. The standard fee for a session for an individual is $200.00. Longer extended sessions, telephone conversations over 15 minutes, report writing and reading, attendance at meetings with other professionals you have authorized, and time spent performing other services you have requested of me etc. will be charged at different rates that have been discussed, unless indicated and agreed upon otherwise.

I accept payment via cash, check, FSA/HSA, and all major debit/credit cards. Visa, MasterCard, Discover, and American Express are accepted. Checks may be payable to “Lee Spencer, M.D., PLLC.”
I am not a contracted provider of your insurance or managed care provider. I am considered an out-of-network provider with your insurance company. If you have a Preferred Provider Plan (PPO), I will provide you with a statement (known as a superbill) which you may submit to your insurance or managed care provider to seek reimbursement of fees already paid.
Under Section 2799B-6 of the Public Health Service Act, health care providers are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give clients who don’t have insurance or who are choosing not to use insurance an estimate of the bill for medical items and services.

You have the right to request a copy of your mental health records.
To request your records, please submit a written request to your treating clinician or to the practice directly. Requests may be made via email, secure client portal, or in writing. Records will be provided in accordance with Texas law and applicable privacy regulations.
