Medical case management is a collaborative process designed to ensure that individuals with disabilities, illnesses, or injuries receive the appropriate medical care they need. I assist in coordinating medical resources and communicating healthcare needs effectively to all parties involved. Through ongoing monitoring and evaluation, we ensure that the care plan is delivering the desired results while promoting cost-effective solutions. I prioritize the well-being of each individual I serve, providing comprehensive support and guidance every step of the way.
The purpose of case management is to support and/or create positive change so the client can experience life more fully. It provides an opportunity to better, and more deeply, understand oneself, as well as any problems or difficulties such as abandonment issues, failure to launch, or sadness due to existential crisis.
